の自分らしい生き方を支えるための外来看護実践上の課題の焦点化と外来看護指針（以下、指針） の考案過程の意義を検討することである。\n　B 病院に外来通院する患者の体験と要望の面接調査と看護師の面接調査から解決すべき課題を明確にした。B 病院の外来師長・内科外来看護師・がん関連認定看護師と筆者は解決すべき課題についての意見交換を重ね、指針を作成した。\n　患者は、告知時は【医療者には、これから延命治療に臨むために心身の安寧が保てる関わりをして欲しいと思う】、外来通院中は〔副作用への対処方法や生活上の工夫について外来看護師からも情報提供したり、経口抗がん薬に変更されても関わって欲しいと思う〕等を要望していた。看護師は＜把握した患者の大切にしていることなどの情報共有や病院内の連携を強化すること＞等を課題としていた。解決すべき課題は『経口抗がん薬治療を受ける患者への看護支援を行う体制を整えること』『患者の大切にしていることや価値観を記載し、患者に関わる医療者で情報共有する必要がある』等4 つに整理できた。指針考案のための意見交換を計6 回重ね、［内服切り替えに対する患者の思いに寄り添い、この時期に患者の大切にしていること・価値観などを把握する］ 等9
