Nursing","bibliographic_titleLang":"en"}]}]},"item_10002_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"　併設病院入院患者で訪問看護が必要な患者に、訪問看護に携わる筆者が退院支援に関わり、在宅療養に向けた支援内容の明確化、併設病院での退院支援の流れの確立、支援体制の変革の検討を目的とした。\n　訪問看護が必要と判断した対象に対し退院支援を実施しその内容を記録する。記録を支援の目的の観点で分析し支援内容を明らかにする。支援後に退院した患者家族に半構成的面接を行い、入院中の看護と退院支援に対する思いを確認し、在宅療養に向けたケアや指導の要望等を分析する。支援の過程で確認した訪問看護導入患者の退院支援の流れを図式化し、併設病院での退院支援体制の変革を検討する。\n　退院支援の実施と分析により、在宅療養を決定する支援、レスパイト先の探索、医療処置の習得、医療物品の準備、退院前の会議等が必要な支援であった。支援後の患者家族への聴き取りから、早期からの在宅サービスの情報提供や病棟での指導の充実等を退院準備として要望していた。スクリーニングシートを用いた患者の把握と退院支援の取組みにより訪問看護導入患者の退院支援の流れは改善し、対象の選定が可能になり、訪問看護ステーションと病棟で協働して退院指導や退院前の準備が実施でき、退院後の状況を共有できるようになった。病院全体としての退院支援手順と退院支援フローチャートの検討・作成に発展した。\n　以上より、在宅療養に向けた支援は、患者・家族の意思決定を支える（入院早期からの支援、在宅サービスの紹介）、療養環境を整える（往診医・レスパイト先を探す､退院後の医療物品準備､医療処置方法の習得）、病院と在宅の関係者が支援の確認と検討の場を持つことが支援内容として重要であると考えられた。また、支援体制の変革には、病棟看護師や訪問看護師、その他関わる病院・在宅での担当者が協働で支援できる仕組みを構築し、病院全体の理解を得ながら運用・検討・修正していくことが必要であると考えられた。","subitem_description_language":"ja","subitem_description_type":"Abstract"}]},"item_10002_heading_23":{"attribute_name":"見出し","attribute_value_mlt":[{"subitem_heading_banner_headline":"原著"}]},"item_10002_publisher_8":{"attribute_name":"出版者","attribute_value_mlt":[{"subitem_publisher":"岐阜県立看護大学","subitem_publisher_language":"ja"}]},"item_10002_source_id_11":{"attribute_name":"書誌レコードID","attribute_value_mlt":[{"subitem_source_identifier":"AA11546486","subitem_source_identifier_type":"NCID"}]},"item_10002_source_id_9":{"attribute_name":"ISSN","attribute_value_mlt":[{"subitem_source_identifier":"1346-2520","subitem_source_identifier_type":"PISSN"}]},"item_10002_version_type_20":{"attribute_name":"著者版フラグ","attribute_value_mlt":[{"subitem_version_resource":"http://purl.org/coar/version/c_970fb48d4fbd8a85","subitem_version_type":"VoR"}]},"item_creator":{"attribute_name":"著者","attribute_type":"creator","attribute_value_mlt":[{"creatorNames":[{"creatorName":"Kato,
