Nursing","bibliographic_titleLang":"en"}]}]},"item_10002_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"　「地域包括ケアシステム」が推進される中で、保健医療福祉サービス利用者が医療機関を退院した後も住み慣れた場所で望む療養生活を続けるためには、利用者ニーズに対応できるよう退院支援に必要な知識・技術を修得し、多職種と連携・協働しながら支援方法を構築していく能力をもつ看護職者の人材育成が重要となる。本学では2004 年度から県健康福祉部医療福祉連携推進課と協働で県内の退院支援の質向上に向けた人材育成の方策を追究している。\n　2004 年度からの5 年間は県の「退院調整と地域連携推進事業」として退院支援の質向上に向けた教育支援に取り組んだ。2009 年から2011 年は看護大学の共同研究事業として、2012 年からは看護実践研究指導事業として、利用者ニーズを基盤にした退院支援の質向上に向けた看護職者への教育支援「退院支援教育プログラム」を策定し、継続的に教育支援に取り組んでおり、2018 年度までの6 年間に当該教育プログラムを修了した看護職者の総数が923 名となった。\n　「退院支援教育プログラム」は、ベーシック研修、フォローアップ研修、アドバンス研修を含み、リフレクションを活用した系統立てられた教育プログラムに沿って学修する人材育成プログラムであり、新たな知見を修得して退院支援に取り組み、リフレクション（振り返り）と取り組みを繰り返しながら課題解決能力を修得するところに独自性がある。\n　アドバンス研修修了者の考える利用者ニーズを基盤とした退院支援のあり方とは、患者・家族の思いに寄り添う意思決定支援、その人らしい生活に向けた多職種連携、病棟で中核となるスタッフへの教育支援、院内の組織的な入退院支援体制の整備等であり、当該教育プログラムは、自らの取り組みのリフレクションを積み重ねながら、利用者ニーズを基盤とした退院支援のあり方を追究し、自施設の退院支援の質向上に中核となって取り組める人材育成の方策であるといえる。\n　今後もアドバンス研修修了者が中核となり、県内の退院支援の質向上に貢献できるよう、人材育成の方策を追究していきたい。","subitem_description_language":"ja","subitem_description_type":"Abstract"}]},"item_10002_heading_23":{"attribute_name":"見出し","attribute_value_mlt":[{"subitem_heading_banner_headline":"Ⅲ.
