Nursing","bibliographic_titleLang":"en"}]}]},"item_10002_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"　医療サービス利用者の望む療養生活が継続できるためには、医療機関における退院支援の質向上が求められる。医療機関の退院支援の質向上を目指すために、A 県内の医療機関における退院支援の取り組みや困難等を含む現状を明らかにし、退院支援の充実に向け取り組むべき方向性という点から課題を検討する。\n　調査期間を2017 年4 月10 日から5 月10 日とし、A 県内の102 の医療機関の看護職者を対象に質問紙調査を行い、看護部長26 名（回収率25.5％）、退院調整看護師22 名（回収率21.6％）より回答を得た。回答の自由記載内容を意味内容ごとに分類した。\n　看護部長の回答結果による院内の退院支援の充実に向けた取り組みは【入院決定時から退院後まで確実な退院支援が実践できる取り組みをする】等の5 つに分類され、スタッフ教育の取り組みは【自施設に即したスタッフ教育を実施する】等の4 つに分類された。退院支援の課題は【病棟スタッフによる退院支援の充実が必要である】等の6 つに分類された。\n　退院調整看護師の回答結果による退院支援の充実に向けた取り組みは、【スタッフの退院支援の実践を支援する】等の8 つに分類され、患者・家族への支援のなかでの困難さは【患者・家族への意思決定支援が難しい】等の7 つに、スタッフ教育の困難さは【退院後の療養生活を見据えた退院支援の実践が難しい】等の4 つに分類された。\n　A
