Nursing","bibliographic_titleLang":"en"}]}]},"item_10002_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"研究目的\n　回復期リハビリテーション病棟において患者・家族の意向を尊重し、退院に向けた方向性を確認しながらすすめることができる退院調整プログラムの開発を基盤として、退院支援体制の充実に取り組む。その取り組みを通じて、退院調整プログラムの意義と退院支援の充実について検討する。\n研究方法\n　在宅療養に向けた退院調整プログラムを考案し、多職種間で共有した後、取り組みを実施する。実施した退院調整プログラムの取り組みに関する成果の把握とプログラムの修正をする。\n結果\n　1．退院調整プログラムについて2 つのパターン（他院からの転院・自施設からの転棟）を考案した。退院調整プログラムには、他院から転院する場合の情報収集方法（病院訪問）、患者・家族を交えた定期的な面談、多職種が参加するケアカンファレンス等を取り入れた。\n　2．退院調整プログラムを実施した事例（5
